U.S. individual or U.S. entity: Form W-9
Non-U.S. individual: Form W-8BEN
Non-U.S. entity (company, partnership, foundation, trust): Form W-8BEN-E
Non-U.S. intermediary (fund-of-funds, custodian, flow-through): Form W-8IMY
